Forms and Instructions
To attend Camp Masterson the
following must be provided:
1. Copy or proof the
sponsoring church has Accident Liability Insurance to
cover accidents and claims while at camp.
2, Each camper must be
listed on the Sponsoring Church's
Application For Church's With Multiple Campers or each camper must be listed
on an Individual Application.
After you complete the application, please print a copy and bring it with you to
give to Camp Director Brother Mike Kelley. Also click on Email to Brother Mike button at the
bottom of the form. By clicking on Email Brother Mike, you will be able to
automatically email the information to Brother Mike, so he can better prepare
for the Sponsoring Churches and Campers.
3. Each camper must have a
Personal & Medical Consent Form.
You can not complete this form on-line, because it needs written signatures on
it.
4. Payment for each camper
attending the camp. Here are the fee amounts.

Special Note:
Friday morning's schedule will remain the same until 9:30 am, when we will
meet for morning Worship. You will be dismissed after this service to
get packed and on the road. No lunch will be served on Friday. |

| We have activities planned for all
ages. Our camp has a full size gym. We will have a basketball
tournament and some volleyball games. We have fun relay races for all
ages, and our most popular attraction, the water slide. We have a
playground which contains swings, a slide and a merry-go-round for the
youngsters. Each participating church is asked to bring the sporting
goods you have, such as balls, bats, etc.. Please be sure that yours are
marked. |

Camp Rules
EACH PASTOR OR ADULT COUNSELOR IS TO SEE THAT THEIR
CAMPERS KNOW AND ABIDE BY THESE RULES.
- Each camper will be responsible for their items.
Mark them in case they are misplaced.
- Keep the camp area neat and clean. No food or
drinks are allowed in the gym, dorms, or Chapel area.
- Avoid handling other camper's bunks or personal
possessions especially wallets or purses.
- Practical joking and horseplay will not be allowed!
- No camper is to leave the camp area without specific
permission from the Camp Director Brother Mike Kelley.
- The "Lights Out" time will be strictly enforced.
- Park all cars, busses, and campers in specified areas.
- No smoking or any other use of tobacco is permitted at
camp.
- Campers are not to enter the area designated for the
opposite sex.
- Campers are to attend all scheduled activities.
- Campers problems are to be handled first by their
counselor; then if necessary, by Camp Director Brother Mike Kelley. Final decision concerning
the emotional, physical, moral, or spiritual problem of the camper will be
made by the camper's Pastor.
ATTENTION PASTORS
Most participating churches
attending Camp Masterson maintain insurance for youth outings on a year round basis. It was decided
that each group coming to camp would be responsible for their own accident
liability insurance. REPEAT: You must provide insurance coverage for
your group prior to camp. You can mail a copy showing current coverage
to Bro. Mike when you mail the Camper Applications as soon as possible to help
with ordering camp supplies.

Dress Codes

GIRLS |
| Please wear dresses to all
classes and worship services, unless special permission is given.
Casual clothes may be worn for activities and free time. Please, no
"SHORT" shorts, mini-skirts, tank tops, or tight pants. No bare feet
outside. Anyone breaking these rules will be asked to change. |

BOYS |
| Shirts and shoes must be worn when
outside the dorm. Pants must be worn to all classes and worship
services. Casual clothes may be worn for activities and free time.

SPECIAL NOTE |
| No clothing may be worn with beer ads or
other suggestive words or pictures. Please observe the Biblical
standard of modesty at all times. If there is any question, please ask
your counselor to help you. |

WHAT TO BRING TO CAMP |
| Bible, pen or pencil, notebook, clothing
for entire camp period, a light jacket or sweater, comfortable shoes, sheets
(beds are bunk style), a light blanket or sleeping bag, and pillow.
Also bring towels, wash cloths, beauty aids, special music, and extra
spending money (for concession stand).
**You will also need old clothes for the water
slide.** |

WHAT NOT TO BRING TO CAMP |
| Radio, t.v., fireworks, pocket knives.
If your unsure, ask your Pastor or Camp Counselor prior to leaving for camp. |
